After the introduction of MALDI-TOF MS, Aerococci continued to be preliminarily identified based on colony morphology and Gram stain morphology. Isolates found to be alpha-haemolytic Gram positive cocci were formally identified using the MALDI-TOF. Aerococcus urinae is a rarely reported pathogen, possibly due to difficulties in the identification of the organism.
Aerococcus urinae (A.urinae) is a rare bacterium that can occur in a urinary tract infection, infective endocarditis, blood stream, and spinal disc infections. A. urinae is a gram-positive coccus occurring in tetrads or pairs, similar to the α-hemolytic Streptococci, which is commonly found in the urinary tract in children and elderly people. However, unlike streptococci, Aerococcus species divide on 2 planes at right angles, resulting in tetrads and clusters on Gram stain (Facklam and Elliott, 1995; Aguirre et al., 1992). The genus Aerococcus was initially described by Williams in his description of Aerococcus viridans ( Williams et al., 1953 ).

The Gram stain should be differential, as A. urinae forms pairs, tetrads, and clusters. However, since A. urinae shows smaller cocci and fewer tetrads than A. viridans does, it could be confused with pediococci or densely packed streptococci or enterococci.
